You call. She says she’s fine. But there was the missed follow-up, the new prescription nobody explained, the pill organizer that doesn’t look quite right. You’re managing work, your own family, and your parent’s care over the phone, and you’re never sure what’s actually going on.
Keystone gives you a registered nurse right here in Jupiter and Palm Beach County who sees your loved one every week, understands the medicine, and tells you plainly what needs attention.
Weekly visits, a medication review every time, follow-ups tracked, and small problems caught early.
We go to appointments with your loved one, ask the right questions, take notes, and explain the plan in plain language.
One person connecting your loved one, your family, and every provider, so nothing falls through the cracks.
Anything interacting? Anything still taken that should have stopped? Refills on time?
Follow-ups booked, attended, and acted on.
Rugs and cords that could cause a fall. Expired food in the fridge. Clutter that makes life harder.
Eating, hygiene, sleep, mood, even feet and nails. The small signs that something has changed.
A trusted local network of physicians, and help booking the visit when something needs a closer look.
A clear update after every visit, so no one is left guessing.
Every family is matched with one Nurse Ambassador. Because each Nurse Ambassador looks after a small number of families, she knows your loved one’s medications, history, doctors, and family by heart.
As Keystone grows, it grows by adding Nurse Ambassadors, never by stretching one nurse thinner.
About 10 families per Nurse Ambassador.
Then we welcome the next nurse.
Every situation is different, but the feeling is often the same: too much to keep track of, and no one person seeing the whole picture.
Adult children caring for a parent, near or far.
Parents of children and teens with special needs.
Adults who want to stay independent at home.
Families facing a change in care.
Florida families have access to programs that can help with care at home, medical supplies, respite, and long-term planning. Most never hear about them, and the ones who do often don’t know where to start.
We help you find out what may apply to you, and we help you through the paperwork.
